I feel you on this but I still don't see the relation with John Q who (according to the movie) went into the hospital with an unloaded gun.  He would of been the only casualty in that situation.  Bump J on the other hand had a gun that could of possibly been loaded(unknown) and probably would of shot someone i.e. the police if need be to make the escape.
